Patient records and clinical notes
Everything about a patient is in one record: their history, their encounters and their documents. Practor builds the invoice from the encounter, and patients read their own record in the portal.
One visit, from the appointment to the invoice.

- Add a patient in a minute A first name, last name, date of birth and email address are all Practor needs to add a patient and book them in.
- Bring your list across Import your patients from a CSV file. Anyone already in your practice is skipped, and a row that fails can be fixed and run again.
- Practor spots duplicates When two records look like the same person, Practor flags them. An owner or admin compares the two side by side and merges them, and a merge can be undone.
You always know who has opened a patient's record.
A practice reaches a patient's record only while the patient is connected to that practice, and nobody at Practor browses patient records. The practice owner sees every time a record was opened in the audit log, and patients see the last 50 times theirs was.
Patients see their own record
A patient you invite signs in from a link in their email, with no password. They see their appointments, invoices and records, and they fill in their own history, allergies and medical insurance, so you don't have to.
- Bookings first, records after ID A patient who books online starts with bookings only. The rest of their record opens once they verify their ID, or you confirm their identity in person.
- Results after you write up A result appears in the patient's portal only once you've finished writing up the visit. Letters and reports you share appear under Documents.
- New patients fill in their own forms Practor sends your intake questionnaire by SMS, or by email when there's no phone number, and adds their answers to the record.
Questions practices ask
Can I import patients from my current software?
Yes. Export your patient list as a CSV file of up to 5 MB with a first name, last name, date of birth and email for each patient, and import the file. Anyone whose email is already in your practice is skipped, and failed rows can be edited and run again. Import patients from a spreadsheet
Can a visit note be changed after the visit?
For 7 days, yes. Finalise the encounter yourself, or Practor finalises the encounter at the end of the week and locks the encounter an hour later. A locked encounter can't be edited or reopened, and the record shows who finalised the encounter and when. Record an encounter
Who can see a patient's record?
Only a practice the patient is connected to, and within that practice, what each person's role allows. A practice can't read records held by another practice, and nobody at Practor browses patient records.
Is every access to a record logged?
Yes. The practice owner sees every record opened, sign-in and permission change in the audit log, with patient data marked, and can export the log as a CSV. Patients see the last 50 times their own record was opened. Read about the audit log
What do patients see in their portal?
Their appointments and invoices, a health summary with their allergies, current medicines and what they're being treated for, and the conditions, results, visits and documents you share. They sign in from a link in their email, with no password. What patients see
How does a patient get access to their record?
Invite them from their record and they get a sign-in link that lasts 7 days. A patient who booked online first verifies their ID against what the practice holds, or you confirm their identity in person. Invite a patient to the portal
Can I export a patient's history?
Yes. Print the medical history or export the history as a PDF, with any filters you've applied. Patients can download a copy of everything on file about them, and that download is logged. A patient's medical history
What do patient records cost?
Nothing. Patient records, encounters and the patient portal are free for everyone in the practice, with no seat charge. The first 10 GB of document storage is included. See every rate
